Accident: Fish Egg Inlet, British Columbia. 17-Sep-1994. The Twin Otter took off from Fish Egg Inlet logging camp for a 16nm VFR flight to Pruth Bay. The aircraft climbed though 100 feet, and just as the flaps were retracting, a violent pitch up was experienced. The aircraft stalled, crashed into the water 100 feet offshore and sank immediately. One member of the two crew and the two passengers were killed. The aircraft was reported as damaged beyond repair. recovered and rebuilt by Ashe Aircraft Enterprises. TSB Report A94P0215.

Note: Although determined to be damaged beyond repair it was subsequently registered to Ashe Aircraft Enterprises in Jun-1998. Additional background on the subsequent transaction to Aviation Québec Labrador in Sep-1998 is required to conclude this airframe’s history fully. The wing set of C-FDMR was noted at Springbank in Jun-2004.
