Accident 03-Mar-1989. On landing rollout, a gust turned aircraft toward a ditch, which it crossed and hit cargo trailer. Minor damage.

Accident: Monument Valley UT 05-May-2000, The flight was departing runway 34 (4,000 ft. x 75 ft., dirt). According to the operator, the airplane encountered 'wind shear' from the right and then from the left. The take-off was aborted. Directional control was lost, and the airplane veered left and hit a dirt berm.

Winds were reported to be from 210 degrees at 10 knots, with gusts to 20 knots. 2 crew and, 15 passengers survived – repaired.

Total time at May-2000 – 39,514 hours.

Accident: Nth Las Vegas Airport, NV. 11-Sep-2005. At approximately 12:43 pdt, the aircraft being operated on a scheduled air carrier flight, exited the runway on landing and came to rest in a shallow ditch. The aircraft came to a stop on a gravel area to the right of the landing runway, 12R, settling onto its left wingtip, and oriented approximately 80 degrees to the right of the runway centreline. Damage was limited to the left wingtip fairing, lower wing skin 24" to 36" inboard of the fairing, outboard left aileron panel and aileron hinge. The left main tire had come off the rim due to side loads, and there was minor damage to the left main strut fairing. The two crew and 17 passengers were uninjured. The wind conditions on runway 12R at the time the aircraft landed contributed to the incident. The captain (pilot flying) reported that on the roll out, with props in reverse, a gust turned the aircraft abruptly to the right. The aircraft continued to the right even as he applied full left rudder and reverse on the left engine. Although the ATIS-reported wind (14015g20) was within the company and AFM crosswind component limitations for the aircraft, it is likely that a gust or shear exceeding this value caused the runway excursion.

Accident: La Fortuna, Costa Rica 02-Jan-2014.  Damaged on landing. The aircraft was arriving on a scheduled flight from San Jose, and there were no injuries.
